In an international week that saw him captain Brazil against Argentina and Peru, the player reportedly had to visit the dentist twice with toothache, according to Sport.
Neymar's appointments have raised concerns in the Barcelona camp ahead of the much-anticipated clash against Real on Saturday.
The 23-year-old managed to get through both games for his country, but the Catalan club are afraid his dental issues could affect him during El Clasico.
The Blaugrana will be reliant on Neymar and attack partner Luis Suarez against their arch-rivals as they have been the Catalans' standout performers this season so far.
It has been reported that Lionel Messi could return to the team for the match after a two-month spell on the sidelines with a knee injury.
