The Buccaneers welcomed goalkeeper Sipho Chaine back from suspension while Evidence Makgopa and Paseko Mako were also recalled to the starting line-up, with Patrick Maswanganyi named on the bench.
SuperSport, meanwhile, made a couple changes from the draw with AmaZulu as Quaicoe and Moralo came in for Lyle Lakay and Christian Saile, who both dropped to the bench along with former Buccaneer Vincent Pule.
Jose Riveiro's charges looked to get off to a fast start, with Relebohile Mofokeng and Mohau Nkota seeing a lot of the ball out wide although the defence were alert to any swift counter-attacks from SuperSport.
After waves of attack, the Buccaneers were in the lead from another academy graduate as Mbekezeli Mbokazi got his first goal in senior football with a well-taken finish in the 18-yard are.
SuperSport managed to equalise four minutes before the interval as Terrence Dzvukamanja found the back of the net from close range against his former employers after pouncing on an error from Chaine.
However, Pirates managed to restored their lead in first-half stoppage time as Makgopa got his first goal of the competition by firing past his Bafana teammate Ricardo Goss.
SuperSport were a lot more livelier in the second period despite ending the first half by conceding, but they were struggling to carve out chances in the visitors' 18-yard area.
The Buccaneers, however, made a triple change after the hour mark as Thalente Mbatha was forced off with an injury and replaced by Goodman Mosele while Maswanganyi and Deano van Rooyen replaced Nkota and Siyabonga Ndlozi.
Time was running out for the Tshwane outfit, who were being restricted to efforts from distance in the closing stages of the second period as the Buccaneers aimed to see out the game.
With the game seemingly headed for a Pirates win, SuperSport popped up with the equaliser in the 91st minute as Gape Moralo embarked on a fine solo run and sent a superb finish past Chaine to send the game to extra-time.
The players were certainly feeling the effects of extra-time, but Pirates should have won it in the final five minutes but Deon Hotto had two efforts saved by Goss after racing clear down the left.
The game would be decided by a penalty shoot-out, but it went to sudden death as Goodman Mosele scored the winning spot-kick after Chaine saved Saile's spot-kick to send the Buccaneers to the final four.
