Los Blancos got off to a poor start when Alaves were awarded a penalty in the fifth minute for handball on Nacho, which allowed Lucas Perez to slot home from 12 yards out.
Zinedine Zidane's charges looked for an equaliser straight away but Alaves shot-stopper Fernando Pacheco produced a good save to keep out Mariano Diaz's header on 15 minutes.
Hazard almost broght the hosts level five minutes later with a fine individual effort, but Pacheco was once again on hand to deny the Belgian superstar.
However, Hazard's evening came to an end in the 29th minute after picking up yet another injury. He was replaced by Brazilian winger Rodrygo.
The battle between Pacehco and Los Blancos continued just before the halftime break as the Alaves shot-stopper pulled off a string of saves to deny Toni Kroos and Lucas Vazquez, with the visitors taking a slender lead into the break.
The hosts made a bright start to the second half, but Alaves were gifted a second goal on 49 minutes when Thibaut Courtois' pass to Casemiro was intercepted by Joselu who fired into an empty net.
Alaves had big a chance to grab a third goal after 64 minutes when Perez raced clear on goal, but Courtois redeem himself by pulling off a good save in a one-on-one situation.
Casmeiro pulled a goal back for Los Blancos with two minutes left on the clock with finish from close range after Vinicius' initial shot was saved by Pacheco.
However, Zidane's men were unable to claw themselves back into the contest in the latter stages as Alaves held on for the win to move up to ninth spot while Real remain in fourth.
