Kylian Mbappe was surprisingly only named on the bench for Los Blancos as Dani Ceballos earned a start in place of Endrick, while the Catalan giants were without the services of top scorer Robert Lewandowski.
Los Blancos coach Carlo Ancelotti was forced to make a change after 11 minutes as Ferland Mendy picked up an injury, with Fran Garcia brought on as his replacement.
Barcelona dominated proceedings in the early stages but had to wait until the 28th minute for the opening goal as Lamine Yamal did well to lay the ball off for Pedri, who sent a superb strike into the top corner from 20 yards out.
Madrid had the ball in the back of the net 10 minutes before the halftime break through Jude Bellingham's strike, but the goal was ruled for an offside on the England international.
Ancelotti responded with a change at halftime, as Mbappe made his way onto the pitch in place of Rodrygo, who had a really quiet first half on the right flank.
The change proved to be an inspired one after Mbappe restored parity for Los Blancos on 69 minutes by firing home free-kick, although Barcelona goalkeeper Wojciech Szczęsny should have done better with the save.
Los Blancos completed the turnaround six minutes later as an un-marked Aurélien Tchouaméni nodded home a corner from Arder Guler.
However, Barcelona were not to be outdone as Yamal turned provider once again, this time playing in Ferran Torres who poked the ball beyond the advancing Thibaut Courtois.
With the game headed for extra-time, Barcelona were awarded a penalty in the sixth-minute of time added on for Raul Asencio's foul on Raphinha, however after the referee viewed the incident on the Video Assistant Referee (VAR) monitor, the decision was overturned, sending the clash to extra-time.
Barcelona did re-take the lead on 115 minutes when Jules Kounde intercepted a pass from Modric before rifling the ball into the far corner from around 20 yards out to seal the title for the Catalan giants.
