The defending champions lost to the Tanzanian Premier League title holders at a packed Mkapa National Stadium in Dar-es-Salaam, recording their first defeat in the competition since 29 November 2019.
Simba opened the scoring in the 31st minute when a brilliant strike by former Sundowns winger Miquissone from outside the box went into the back of the net via the crossbar.
The Tanzanian side dominated for large parts of the first half, with Mohamed El Shenawy forced into several good saves before the break.
Al Ahly fared a bit better in the second stanza without really creating any feasible chances. Mosimane reacted after 60 minutes, replacing Mahmoud Kahraba and Walter Bwalya with Mohamed Magdy Afsha and Amr El Solia.
However, his side continued to struggle to create chances, with Simba remaining in control of the game. The South African tactician reacted with another substitution, replacing Akram Tawfik with Mohamed Sherif after 78 minutes.
Al Ahly's Aliou Dieng found himself in a decent position with five minutes to play, but his header sailed over.
The visitors thought they had equalised in injury time but a brilliant save by Aishi Salum Manula prevented the Egyptian giants from finding the back of the net in the 91st minute.
This is Mosimane's second defeat in his 26th game in charge of the "Club of the Century", with Simba moving to the top of Group A on six points and Al Ahly remaining on three.
Meanwhile, Vita Club beat Al Merreikh 4-1 in the other group fixture, seeing them post three points on the log as well.
