A hat-trick from former Fulham striker Aleksandar Mitrovic helped Al Hilal to an emphatic 3-0 win over the Iranian giants.
The Brazil superstar was introduced in the second half, having only recently returned from a lengthy spell on the sidelines, but he was forced off before full-time due to injury.
The 32-year-old looked enraged as he took his place back on the bench, throwing his boots to the ground before receiving treatment from Al Hilal's medical staff.
Speaking after the game, Neymar revealed the extent of the pain he was feeling in his hamstring.
"I felt like a cramp but very strong! I will do exams and hope it's nothing more," he said, according to Fabrizio Romano.
"It's normal for this to happen after one year, the doctors had already warned me, this is why I need to be careful and have more minutes to play."
The former Paris Saint-Germain and Barcelona winger will now be assessed by Al Hilal to determine the seriousness of his latest injury.
The club are likely to take caution given Neymar's history, with the Brazilian's fitness woes having greatly impacted his time in Saudi Arabia.
